Angie Aboutalib, MD
Dr. Angie Aboutalib is a hospitalist and the National Director of Quality and Education for US Acute Care Solutions - Integrated Acute Care. She is also an Adjunct Clinical Assistant Professor for Lake Erie College of Osteopathic Medicine, patient experience faculty member, and a teaching attending for both medical students and physicians assistants. She serves as the Chair of the National Peer Review Committee, board member of Diversity and Inclusion, and member of the Women’s Resource Group. Prior to her current role she was the Director of Clinical Operations for Integrated Acute Care. She is a member of the American College of Physicians and Society of Hospital Medicine.
Christina Baum, RHIA, CPMA
Christina Baum, RHIA, CPMA is a Senior Clinician Documentation Educator at US Acute Care Solutions. She obtained a Bachelor of Arts in Psychology–Community Clinical from Walsh University and an Associate of Applied Science in Health Information Management from Stark State College. While at Stark State College, she was the President of the Student Health Information Management Association and held the distinction of being a member of Phi Theta Kappa Honor Society. Christina’s duties include educating emergency medicine, hospitalist, intensivist, obstetric and post acute care clinicians on documentation requirements and guidelines. She also creates and maintains superbills for clinician utilization, and oversees auditing of provider documentation and billing. Additionally, she leads a robust team that monitor coding and billing industry trends and updates. She is an active member of AHIMA and AAPC.
Jestin Carlson, MD, MS, MHA
Dr. Jestin Carlson is an emergency physician and serves as the National Director of Clinical Education at US Acute Care Solutions. He is also the Program Director for the Saint Vincent Emergency Medicine Residency and Vice Chair for the Institutional Review Board for the Allegheny Health Network. Dr. Carlson has a focus on resuscitation research and education with over 150 manuscripts published in peer-reviewed journals and currently sits on the International Liaison Committee on Resuscitation/American Heart Association First Aid Task Force and the American Red Cross Scientific Advisory Committee. Dr. Carlson earned his medical degree at State University of New York at Buffalo and completed his emergency medicine residency and a research fellowship at the University of Pittsburgh, and Master of Healthcare Administration from Gannon University.
Orion Colfer, MD
Dr. Orion Colfer is an emergency physician and the National Director of Patient Experience and Director of the USACS Scholars Program. He is also the National Director of the Center for Emergency Medical Education. He joined USACS in January of 2002. Prior to joining USACS, he worked at the Grand Strand Regional Medical Center in Myrtle Beach, South Carolina where he was a staff emergency physician. He completed his emergency medicine residency at the University of Virginia, where he was chief resident. He is board certified in Emergency Medicine and is a member of the American College Emergency Physicians. He completed the USACS Scholars Program in 2003.
Casey Collins, MD
Dr. Casey Collins is an emergency physician and the Chief Documentation Officer of Integrated Acute Care for US Acute Care Solutions (USACS). He has served as a member of the Bylaws, Utilization Management, and PCI Performance Review Committees at Carroll Hospital Center, MD, in addition to serving as a Code 44 Physician Advisor. He has provided documentation lectures at EDPMA and Maryland ACEP. Dr. Collins has been a guest speaker for the University Hospitals EM Resident Mentorship Program as well as a recurring speaker for the Cullman Emergency Medicine Fellowship. Prior to joining USACS, he worked as the National Senior Director of Clinical Documentation Integrity for Alteon Health. He is a fellow of the American College of Emergency Physicians and a member of the Health Care Compliance Association. He earned his medical degree at the Pennsylvania State University College of Medicine and completed his emergency medicine residency at Maricopa Medical Center in Phoenix, Arizona.
Dusty Otwell, Esq.
Dusty Otwell, Esq. is Vice President for Claims and Risk Management at US Acute Care Solutions. He also serves as Chief Operating Officer and Corporate Secretary for Clinician Assurance Risk Retention Group, the self-insurance carrier for USACS. In addition he is a Senior Risk Management Consultant for ProAssurance Corporation, staff attorney for Maynard, Cooper & Gale, P.C., and staff counsel for Medical Reimbursem*nts of America, LLC. Dusty is responsible for industry-leading results on malpractice risk management, consistently achieving claims rates less than half of comparable benchmarks. He is a faculty member of the Center for Emergency Medical Education and teaches the High-Risk Emergency Medicine Course. He is a board member of the Texas Alliance for Patient Access, a volunteer for the Lawyer Assistance Committee of the State Bar of Georgia, and was an Advisory Board Member of the Austin-Travis County EMS. He is licensed in Georgia, Tennessee, Alabama, and Texas, and holds a corporate counsel registration in Ohio.
The AMA and CMS (Medicare and Medicaid) Have Mandated New Charting Guidelines. Are You Ready?
ED Charting: In the Beginning
In 1992 the AMA developed very specific charting guidelines that required clinicians to document designated items related to the CC, HPI, PMH, ROS, PE and Medical Decision Making.
Medicare Adopts AMA Charting Guidelines
The Center from Medicare and Medicaid Services adopted these guidelines as did most other insurers as the basis for determining the level of service (99281-99285) and subsequently, payment.
As of January 1, 2023 – No More H/P Requirements
With regard to the history and physical, now clinicians themselves determine what an appropriate history and physical is given the problem at hand, while still being mindful of medicolegal and quality concerns.
As of January 1, 2023 – Medical Decision Making Is Everything
As of January 1, 2023, the previous charting guidelines are GONE, except for Medical Decision Making, as they relate to assigning levels of service. All that matters in this regard now is Medical Decision Making.
The Specifics of Medical Decision Making Are Complicated
The elements of Medical Decision Making have been specifically defined and augmented and are relatively complex – and you’ll need to know the changes cold when you hit the bricks running on Sunday, January 1, 2023.
